Este filme é uma continuação e conclusão do anime de 2003 de Fullmetal Alchemist. A história passa-se dois anos depois de Edward ter ido para o nosso mundo. Este vive em Munique juntamente com uma pessoa muito parecida com o seu irmão Alphonse, que lhe irá ajudar a voltar para o seu mundo.
A ação começa com Jodorowsky – guru numa espécie de rito iniciático. Em seguida somos apresentados a um homem a quem chamam Ladrão, cuja figura lembra a de Jesus Cristo. Ladrão vaga pelas ruas do México quando é crucificado e apedrejado por um grupo de crianças. Nesse momento ele inicia uma verdadeira amizade com um anão que será um dos poucos a acompanhá-lo em sua saga.
Depois de um prisioneiro misterioso conseguir fugir de uma prisão na Central, os irmãos Elric seguem o seu rasto. A busca leva-os a Table City, no país de Cleta, onde Alphonse salva uma rapariga chamada Julia que também é procurada pelo prisioneiro de quem andam atrás.

Etienne (Guillaume Canet), um jornalista que pretendia escrever um livro sobre Vidocq, passa então a seguir os últimos passos de seu personagem e acaba tendo surpreendentes revelações em sua viagem ao submundo de Paris.
