כמו בשני הסרטים הקודמים, גם הפעם העלילה מתמקדת ב"ארט הליצן" שבסרט החדש משחק את סנטה קלאוס למען תושבי וילדי מחוז מיילס, והרעיון שלו לחגוג את שמחת החג כוללת באופן מפתיע – רצח, מהומה ובעיקר הרבה דם.

אחד המותחנים המפתיעים והמצמררים של העת האחרונה מספר את סיפורו של סטוץ' לילי שהופך בין רגע למסע רציחות של רוצח סדרתי מטורף.

רוברט מאקול הוא לוחם קומנדו לשעבר שביים את מותו כדי לחיות חיים שקטים בבוסטון. כשהוא חוזר בו מהפרישה שלקח לעצמו כדי להציל נערה צעירה העוסקת בזנות, הוא מוצא את עצמו מתעמת ונלחם פנים מול פנים עם ארגון רוסי חסר רחמים ואלים. כשמאקול נוקם את כבודם של חסרי הישע הניצבים חסרי אונים מול אלימות משתקת, התשוקה של מאקול להילחם למען צדק גוברת על רצונו לחיות חיים אנונימיים ושקטים. הוא מחליט כי ייעודו בחיים הוא לעזור לכל חסרי האונים, הנתקלים במכשולים מאתגרים - הוא מחשיב את עצמו כנקודת השוויון.
