Walter Sparrow, petit employé dans une fourrière d'animaux, voit son existence chamboulée quand Agatha, son épouse, lui offre un étrange roman, « Le nombre 23 ». Dès les premières lignes, Walter a l'impression que l'histoire racontée dans le livre est la sienne. Il s'identifie immédiatement à Petitou, le héros, et commence à partager ses angoisses et sa fascination pour le nombre 23 : celui-ci renfermerait un pouvoir caché.
