A tak se Rocky se svou ženou Adrian a synem stěhuje tam, kde kdysi začínal - do zašlé a levné čtvrti v jižní Philadelphii. Musí však překonat hluboce zakořeněnou zatrpklost a hořkost, která roste s tím, jak jeho nový svěřenec - mladý boxer Tommy Gunn získává na kvalitě. Když se Tommy obrátí proti svému učiteli a veřejně ho zesměšní, je Rockymu jasné, že ho čeká ještě jeden souboj...

Tam ho následuje Otsu a později i Akemi, obě do něj zamilované. Rok končí, když Takezo pomáhá vesničanům proti bandě lupičů. Žádá Otsu o odpuštění a přijímá její lásku, pak se vydává přes vodu na ostrov Ganryu, kde se koná jeho poslední soutěž.

V něm žije i krásná náčelníkova dcera Loana (Carole Landis), která se o Tumakov stará. Kvůli neshodám musí po čase oba kmen opustit a Tumakov se rozhodne vrátit do svého rodného kmene, kterému chce ukázat, že lidé mohou přežít i v míru. Spolu s Loano je čeká tvrdý boj o přežití a za sjednocení obou kmenů; to vše v době krátce před ničivým výbuchem sopky.
